Incident log

Record what happened, add later updates and help the next shift find the facts.

Reviewed:

Find incident and customer records

  1. Open Incident log for the selected establishment. Your administrator controls who can read, create, correct and print records.
  2. Choose Incidents for reports, Customers for customer profiles, or 86’d for customers currently restricted from the establishment. “86’d” means the customer is not permitted to return under the recorded restriction.
  3. Use Incident date range, set Start date and End date and select Apply dates when looking for reports from another period. Use Search customers for a name or description. Select Load more for additional results.
  4. Open the record to read its details, updates and available actions. Check the establishment and dates before assuming a report is missing.

Write an incident report

  1. Select Incidents → New incident. Enter a specific Title, check Occurred at and write Incident details. Use the establishment’s timezone for when it happened.
  2. Describe what you observed and what action was taken. For example, “A guest dropped a glass at table 4. Staff closed the area and cleaned it.” Separate facts from anything reported by someone else.
  3. If relevant, use Customers involved to select existing customers or Add a new customer. Review names and descriptions so you do not associate the wrong person. You can include up to 10 customers.
  4. Select any evidence files, review the report and attachments, then select Save incident. Wait for Incident saved and check the saved report.

Correct a report or add new information

  1. Open the report. Use Edit incident to correct the original report when you have that action. Review your changes and select Save incident.
  2. Use Add update for something learned later. Enter the Update, add any new customers or evidence and save. The update is added without replacing the earlier account.
  3. Open View history to read saved revisions, who made them and when. Use Earlier history for older entries when offered.

Creators with correction access normally have 60 minutes after creation to correct their own report. Edit any incident permission allows later corrections. Adding an update cannot remove earlier evidence or customer associations. Older records may start with a baseline when history was introduced, so earlier overwritten wording may not exist.

Review a customer or change a restriction

  1. Open Customers and select the person. Use Edit customer profile when offered, review the name, description and optional photo, then Save customer profile.
  2. If you have permission to change restrictions, select Change 86 status. Choose Active / Not 86’d, Temporarily 86’d or Permanently 86’d. For a temporary restriction, set a future Expires at.
  3. Select Review change, check the person, status and expiry, then Confirm change. Confirm the saved status before acting on it.

Expired temporary restrictions leave the current 86’d list. A saved or printed list may be out of date; check the current record. Customer profiles are separate from employee accounts.

Open evidence and export a report

  1. Open the report’s Files or the customer’s attachments. Select Open file to retrieve an available file, then use its media or download controls.
  2. For a report, select Export / Print incident report or Export / Print customer report. For current restrictions, open 86’d and select Export / Print current 86’d list. Each action requires its own print permission.
  3. Read the preview and included records. Select Export CSV to download the data or Print / Save as PDF to finish in your browser’s print window. A PDF destination depends on your device.

Evidence is listed in reports; private audio and video are not automatically embedded. Download needed files separately. Keep reports confidential and check current records before relying on a saved copy.

Remove a report when authorized

  1. Open the report and select Remove incident if offered. Read the confirmation and enter Removal reason.
  2. Select Confirm removal only for the intended report. It leaves ordinary views, but the report, evidence and history are retained.
  3. Current administrators can use Include removed reports to read retained removed records. Removing a report is not permanent deletion.

If records will not load or an action is missing

  1. Check the establishment and date filters. Use Refresh incident records or Retry if offered. A failed load does not mean there are no reports.
  2. If edit, update, restriction or print controls are missing, ask your administrator to review the relevant permission. Your correction window may also have ended.
  3. If Incident log was disabled, authorized readers can find Retained incident records · Read only in Tools & settings. For an app error, contact Help & feedback without including confidential incident details.
